Blood test, electrocardiogram (EKG), eye screening, physical examination, cardiology evaluation, cancer screening/prevention education, dental screening, physical therapy, pharmacy, dietary and weight management and mental health counseling for people of age 40 and above who are uninsured or underinsured. No paper copies or email copies are accepted and no walk-ins are allowed due to logistical and legal issues. To fill out the registration form, please visit organization website at www.IHCNJ.org. Please follow the instructions to complete the registration form and submit the form online no later than Friday, September 9, 2022. |
